IAHCSMM members are invited to verify their own personal information by accessing the Member Portal of the website. We are currently updating our software to better serve you online. During this update, please complete the following form and an email verification will be sent to the address you indicate. Please note this email verification is only valid if matched against a current membership/certification card presented to you by the employee. The email verification is only a secondary form of certification identification, and is not intended to be the primary source when verifying certifications of hospital personnel. Email verifications will be sent within 1 business day of receipt of request, only to valid email addresses provided and to person’s requesting verification with a Human Resource or Supervisory/Management Title listed below: |
